Community Access Services is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to support individuals with intellectual and developmental disabilities in a group home setting. Key Responsibilities of an LPN:
  • Provide direct support to individuals in meeting their personal care needs, including lifting/transfers, hygiene, all activities of daily living (ADLs), and medication administration per physician orders and CAS procedures.
  • Work collaboratively with Support Staff to ensure consistent, high-quality care is provided at all times
  • Review and assess daily needs by reading communication logs, medical progress notes, and other documentation; follow up appropriately based on current medical and support needs
  • Adhere strictly to protocols outlined in medical support plans, individual support plans (ISPs), and Oregon Administrative Rules (OARs)
  • Assist with meal preparation and ensure compliance with dietary plans and physician or dietitian recommendations
What We’re Looking for in an LPN:
  • Active LPN license in good standing in the state of Oregon
  • Proficient knowledge in tube feeding, care of J & G tubes, nebulizer treatments, IV maintenance, and familiarity with various pumps.
  • Competency in airway maintenance, trach care, ventilation, and suctioning.
  • Strong understanding of medication administration protocols and awareness of potential side effects.
  • Current CPR/first aid certification (BLS preferred)
  • Must meet eligibility requirements under a national criminal background check
